Hi My,
Try to clean your cookies and then go back to it. That happened to me as well – what I did was I originally clicked on the link – to get the business cards – then I logged off and then when I tried logging back in after I had logged out I did not see the link there on the page for the form. So I then cleaned my cookies out and it was there again.
