I filled out the survey and when I was finished a form came up asking for my e-mail address and mailing address. After filling it out and submitting it said my sample would be arriving in 4-6 weeks (or something along those lines). I am using firefox so not sure if it’s your browser or they fixed it as I just filled it out a few minutes ago.
