   After I filled out, noticed the following: “The first 2000 to enter beginning 10:00 AM ET 9/12/11 will receive the sample”. 9/12??? Is this an old one?

   I saw the same thing, so I didn’t bother filling it out. If it started in September 12, it’s reached the 2000 for sure.
