I have tried in both Chrome and IE, neither one will submit, I fill it out and hit submit and nothing ever happens.

Hi Evin, When you submit the form at the very top of that form it will say “Thank you for submitting your information! Your sample will be shipped to you within a week…” that means it submitted through. I hope this helps. 🙂
