If you go to the very bottom of the page…the official rules state:

First come, first served: First 500 entries will get one sample-size K-PAK® RevitaLuxe™ (1.7 oz.), starting from September 1, 2011 (12AM PST)

it went back online but did not confirm if my info went thru. hopefully i get a confirm email. otherwise how will i know?

  I tried a few times and it just sent me to the FB page. Finally, after I filled out the form again, it said something like “this address has already been submitted” Hoping that means it worked!!
