   There appears to be a problem with the form. I have filled it out twice and it rejects my tel. # (which I’ve put in different ways) and the screen won’t advance….

     I had the same problem. I almost gave up. I just saw someone’s post in the comments, it said you must use the dashes in your phone number. I did that and it advanced. Good luck

   I didn’t have a problem with the form and I filled it out for me and my mom. We put the phone number with the area code and number with dashes between. Is that how you did yours? It worked with a landline or cell number. Or is your # a US number?

     yes, that’s how I did mine. It didn’t matter which type of number (landline/cell) the
     key to it was entering the ‘DASHES’ .

   Yes, I put in the dashes and it went through no problem. Good luck, everyone. I’ve used Biofreeze before and I loved it, it feels wonderful on a sore back.
