When you click on the link you need to let the page load for a minute. Then you will see in a box “Are you at risk of getting Gingivitis?” Right under that it will say take the quiz- click on that and when you answer a few questions the form will pop up.

Hi Mike, that is what I was already doing. when it popped up a box after I took the quiz that said “good news….request sample” I clicked on request sample button & it is still saying sorry, the url you are looking for isn’t here. do you have the direct link? if so thanks a bunch!

Oh, and that was so funny, one of the choices was “I order all samples from Walmart.com”, did anyone answer that honestly, lol!?
It looks like they send a little tube of the Pro-Health, but maybe just a one-use siz of the Crest w/Scope.
Oh well. I guess if it’s a little one use packet, it will be convenient to keep in your purse for when you just might need it! The little tubes I like to take with me on vacation/travel. But I am gonna keep the little packet, if that’s what it is, in my purse 😀
